Please provide the identification data of the person answering the survey (name, telephone, fax and e-mail).

• Legal status
• Religious denomination or other character

4. Activities and services offered by the centre

• Specific centres
• Levels of education
• Number of pupils and units by level
• Subsidised units (for non university centres)
• Complementary services, after-school or extension activities

5. Personnel of the centre

• Personnel of the centre by category or area of activity
• Teachers by level of education
• Hours worked by teachers (paid and unpaid) by level of education

6. Personnel costs

• Expenditure on teaching personnel by education level
• Non-teaching personnel costs.
• Other personnel costs.

7. Current account expenditure on goods and services

• Expenditure on educational activities
